Introducing Chronic Care Reform

68% of Medicare patients have 2 or more chronic conditions like diabetes or heart disease-- accounting for 93% of all Medicare spending.  Senators Ron Wyden, D-Ore., and Johnny Isakson, R-Ga., and Representatives Erik Paulsen, R-Minn., and Peter Welch, D-Vt., introduced the Better Care, Lower Cost Act to provide better care at a lower cost for the millions of Medicare beneficiaries living with multiple chronic conditions.  AARP, Emory Healthcare, Montefiore, Blue Cross Blue Shield of Massachusetts, Cigna-Healthspring and Health Quality Partners all attended the event in support of improving chronic care for Medicare beneficiaries.
